Home care service

Our Disabled Children's Team is a front line assessment, case management team which specialises in providing support and services to disabled children and young people across the County Borough of Bridgend. This includes children who are looked after.

Support worker

Community reablement team

20 hours per week 

£10,569 per annum

The Community Resource Team is an integrated health and social care service which provides a range of short-term interventions to people in their own homes to promote their independence.

You will play a key part in supporting the CRT Services achieving its aims to support people to continue to live in their chosen environment by preventing avoidable hospital or care home admission and to facilitate early discharge from hospital. You will be providing Domiciliary & Therapy support across the service.
