Community Disability Rehabilitation Team
The Community Disability Rehabilitation Team (CDRT) is a highly specialised health and social care service funded in partnership by Bridgend Local Health Board and Bridgend County Borough Council.
The team consists of an Operational Manager, Occupational Therapist, Physiotherapist, Speech and Language Therapist, Social Worker, Nurse and Administration Assistant.
We work with adults who have complex chronic conditions and their carers to:

Provide advice, support, training, and treatment |
|
Identify specific goals to maintain or maximise independence |
|
Work towards achieving these goals |
|
Provide information about other resources and services |
|
Help to co-ordinate ongoing care with other professionals |
Service Access Criteria
People who would like to access the service will be:
|
Adults who are resident within the county borough of Bridgend | ||||
|
Diagnosed as having a complex chronic condition such as: | ||||
|
Sudden onset conditions Acquired brain injury, stroke or spinal cord injury followed by a partial recovery. |
||||
|
Intermittent and unpredictable conditions Epilepsy, multiple sclerosis where relapses and remissions lead to a marked variation in care needed. | ||||
|
Progressive conditions Motor neurone disease, Parkinson’s disease, progressive multiple sclerosis leading to increasing independence for support or care from others. | ||||
|
Stable Conditions but with changing needs as time progresses Cerebral palsy, Spina Bifida or Post Polio Syndrome. | ||||
|
Ready for safe hospital discharge (*if currently in hospital) with a package of care already in place prior to acceptance by the team. | ||||
|
Able to be medically managed by primary care (*if living in the community). | ||||
|
Motivated and able to participate in a rehabilitation programme. | ||||
|
Requiring support from more than one professional within the team. | ||||
|
Require Social Work input to arrange a package of care to support discharge from a regional hospital. | ||||
|
Referrals can be made through your health or social services professional or from the specialist voluntary sector. | ||||
