HOSPITAL DISCHARGE SERVICE
Age Concern Morgannwg’s Hospital Discharge Service provides temporary extra help and support to the residents of Bridgend County Borough aged 50+ when they are discharged from hospital. It is a free service.
What support can they provide?
Some of the tasks that we assist with include:
• Collection of shopping and medication
• Benefits advice and form filling
• Assistance with bed changing/laundry
• Emotional support
• Information/liase with other agencies
• Support to carers
Who will provide the support?
Age Concern Morgannwg’s trained support workers will visit clients as soon as possible after discharge and a plan of support will be agreed.
The support will continue for up to an hour a week for around six to eight weeks, if required, and will be reviewed regularly to ensure the service meets the needs of the client. Age Concern Morgannwg’s Hospital Discharge Service aims to complement the services provided by other agencies upon discharge from hospital and is able to liase on behalf of the client with these other services.

What are the criteria for referrals?
Anyone can refer to the Hospital Discharge Service.
The criteria for referrals are:
Anyone aged over 50 who have been discharged from hospital within the last 13 weeks who:
• Lives alone or will be alone during the day
• Lives with the carers who may need support themselves
• Is frail, housebound or have poor mobility
• Is deemed ‘at risk’
• Refuses other support but is prepared to accept our service
• Has any chronic illness or disability
• Is in need of emotional support
• Is in need of help with day to day tasks |
